We know your time is valuable to you. Our electronic check in system, Phreesia, enables parents to complete the check in process prior to arriving in the office for their appointment. By doing this you can update your demographic information, make any necessary changes to social & medical history, complete various questionnaires based on appointment type such as the Ages & Stages Questionnaire, MCHAT, PSC-17, Vanderbilt forms, CRAFFT, etc, and sign any office policies and procedures.
